All members must submit this form to request Club Access Credentials. There are separate forms for Full Household applications and Individual applications. We prefer you use the online form at the bottom of this page; submissions go directly to management. A PDF version is also available below for download. Completed PDF forms may be dropped off at the Clubhouse or emailed to blackstoneclubhouse.ca@fsresidential.com.

Once your application and documents are approved, a staff member will issue your credentials and take a photo for our records. The Clubhouse team processes new Club Access Credentials daily from 10:00 AM to 8:00 PM. Homeowners will receive an email with the website password to access the HOA Documents section.
